  • Order a base form (ready-made, new, or existing).
  • Add form fields or features.
  • Pricing is per field. So if you need 3 check boxes, update the quantity at checkout to 3.
  • Checkout and pay with a credit card, or PayPal. A PayPal account is not required for credit card orders.
  • Include instructions for your form design in the notes of the order form.
  • Email meldridge@ edocdesign.com with any supporting documents, like forms and logos.
  • You will receive a confirmation email with any questions from our form designers.
  • Your completed custom form will be emailed to you within 2 business days (as soon as 2 business hours).

    issues, or refund your money if a fix is not possible.
New Form, $35 per page
Create a new form. Includes up to 40 text fields. Be sure to include form design instructions in your email, and add/pay for any features beyond text boxes.
Existing Form: Add Fields, $20 per page
Add up to 40 text fields to your document. Includes conversion to PDF from most formats. Does not include resizing or editing the original document.

Complex calculations are multi-step functions that require hidden fields, and so on. JavaScript not included--inquire for pricing.

An import button can fill your form with data from an fdf file.
Conditional Formatting, $1.35
Conditional formatting is used to hide zero values, change text color upon a negative calculation, etc.

Export Button, $3.00
An export button can save your form data to an fdf file. Will not work with Adobe® Reader®.
Edit Original Document, $10.00
per page.
If you need changes made to your original form before we add the form fields, please order this service. We can edit or move text and graphic, as long as the orignal document has computer readable text in a standard Windows font like Arial , Times, or Helvetica.
Other Action Button, $3.00
Any menu action available in Acrobat can be added as an action button. Many menu actions require the full version, and will not work with Adobe® Reader®.
Add Stamp $3.00
Add a stamp to your PDF document. Any image file can be added as a stamp.
Add Watermark $3.00
